Comprehensive drug testing for CDL and non-CDL drivers, going beyond DOT testing with fleet-wide coverage, zero administrative lift, and zero testing liability.
Substance-use liability is one of the most undermanaged risks facing small and mid-sized trucking companies today. A single positive test mishandled, a missed prescription interaction, a non-CDL driver involved in an at-fault accident, or a chain-of-custody failure on a DOT random can expose you to FMCSA enforcement action, nuclear-verdict litigation, workers’ compensation claims, wrongful-termination suits, and skyrocketing commercial auto premiums.
For a fleet of 5 to 100 trucks, one such incident can be existential.
Most drug testing programs were not built to address this. They satisfy the minimum DOT 5-panel for CDL drivers, but leave non-CDL drivers, dispatchers, yard hostlers, mechanics, and light-duty delivery drivers under 26,001 lbs. entirely uncovered. They also ignore many legal, illegal, and prescribed medications that, alone or in combination with other substances, can affect performance behind the wheel.
The result is a fleet with a substantial liability blind spot.
The Family Wellness Box was built specifically to close those gaps for small and mid-sized trucking operations. We deliver comprehensive, affordable, confidential, and convenient trucking company drug testing and administer the entire process, so you carry no operational lift and no testing liability. The Family Wellness Box adheres to all HIPAA requirements.
The Family Wellness Box conducts and administers every component of testing, including collection with or without observed urine samples, chain of custody, lab analysis, PhD-reviewed results, and reporting. All tests are conducted in a CLIA-certified lab.
You initiate the order. The Family Wellness Box handles the rest.
In the event of a disputed result, The Family Wellness Box has an end-to-end process for addressing the dispute and further confirming outcomes. Your testing-related liability, for both CDL and non-CDL drivers, is effectively zero.
This helps trucking companies manage driver testing with less internal burden and stronger documentation.
The Family Wellness Box tests 74 substances drawn from a 12-drug cup.
Standard DOT tests screen for 5 basic drugs, but they miss many drugs of abuse, illegal and legal substances, and prescribed drugs such as antidepressants, anti-anxiety medications, other mental health-related medications, opioids, mushrooms, Kratom, sleep aids, and many more.
These substances, alone or in combination with other substances, can slow reaction time, impair judgment, and put a driver behind the wheel of an 80,000-pound vehicle in a state no insurer wants to defend.
Our LC/MS, Liquid Chromatography Mass Spectrometer, lab tests for these substances, and every result is reviewed by a PhD with decades of experience on the instrument.
The enhanced visibility this service provides helps prevent incidents before they happen.
Identifying the presence of legal and illegal drugs and compounds, drug-related contraindication or interaction before a driver climbs into the cab, or before a non-CDL technician operates a lift gate, forklift, or company vehicle, is information that is otherwise difficult to source.
This is exactly the kind of insight that turns drug testing into a real risk-management tool. For a trucking operation, preventing one at-fault crash can mean the difference between a renewable policy and a non-renewal notice.
Commercial auto and workers’ compensation carriers increasingly factor substance-use testing into underwriting. Most workers’ compensation carriers offer credits, typically 5%–10%, for employers maintaining a documented drug-free workplace program.
Several states, including Florida, Ohio, Tennessee, Virginia, Alabama, Georgia, Mississippi, South Carolina, and Arkansas, among others, mandate the credit by statute.
For a fleet, the savings on premiums alone can more than offset the cost of The Family Wellness Box program, and a clean loss-run history compounds the benefit at every renewal.
Orders are placed on this site. The box arrives with securely packaged cups or cups, all collection materials, and prepaid return shipping.
Standard delivery is typically within 24 hours at no charge, and guaranteed overnight delivery is available for a fee. Samples ship back via UPS at no charge. The sample envelope can be dropped at any UPS location, which is searchable from our site.
If an observed collection is needed, it can be added at the time of order. The 74-substance lab results return in 24–36 hours via encrypted email.
Cost and convenience are unmatched.

Samples, including observed samples, are collected at your location, wherever the driver is, or at another location where you want them tested, not at a clinic across town.
You save the deadhead miles and out-of-service hours of sending a driver to a collection site, and the driver gets a more confidential experience.
For non-observed urine samples, the driver provides the sample, and immediate results are visible on the cup for 12 commonly abused drugs.
A small portion of the sample is transferred to a vacutainer, sealed in a zip-lock bag, placed in a waterproof envelope with the return shipping label affixed, and dropped off at UPS.
Lab results follow by encrypted email, usually within 24–48 hours.
For observed urine samples, The Family Wellness Box sends an observer, typically a licensed Paramedic or Nurse, to the driver’s location for an additional fee.
All observed collections follow strict chain-of-custody protocols.
The driver is instructed not to open the cup before providing the sample. After overseeing the collection, the observer takes custody immediately and delivers the sample to UPS.
If the driver claims a discrepancy, a re-test is conducted within 24 hours and must include an observer, regardless of how the original sample was collected.
If the discrepancy is confirmed, and the original test costs are refunded.
This process helps fleets address disputed results with a clear, consistent, and documented procedure.
