Instructions For Providing an Observed Urine Sample
Please call Customer Service at (888) 420-0990 to schedule your observed urine appointment after you have purchased it on the website. If you do not have a cup, please purchase one on The Family Wellness Box website. Take the cup sealed in in the self-contained packaging it arrived in, the vacutainer, return vacutainer bag, disposable glove, return envelope and return shipping label to the appointment. The observer will give you further instructions at the appointment.
If the packaging surrounding the cup has been damaged or opened or if any of the other required items described above are not brought to the appointment or if you are more than 5 minutes late then a new appointment will need to be purchased and scheduled.

Order and Receive
Order and ReceiveSelect a one, two, three or four-cup box with increasing discounts and use them for up to one year after ordering. 2-3 day shipping is free but you will have an opportunity to pay for expedited shipping.
Collect and Package
Collect and PackageYour order will be delivered in a plain white box with everything that you need to collect sample and with a return shipping label.
Ship it Back
Visit our website and find the closest of many UPS drop locations to return your shipment.
- No Saturday or Sunday Deliveries.
- Cups must be used within one Year.
- In the case of no delivery, call customer service.
Instructions
Instructions- Remove the cup from the sterile packaging.ve the top and provide the urine sample.
- Replace the top firmly on the cup.
- Peel off the side cover around the cup to review the results related to the 12 drugs that are listed on the cup.
- The 12 substances indicated on the cup are listed below
- Cup results are preliminary and fractionally accurate compared to lab testing. Positive results will be confirmed by the lab which tests 74 substances.
- Even if the cup does not indicate the presence of a listed substance the lab test may indicate the presence of one or more of the substances.
- After securing the top back on the cup peel the sticker off the top of the cup and insert the rubber end of the vacutainer into the hole.
- Press it firmly on the needle until the urine begins to fill the tube, then lift it off the needle.
- Only a small amount of urine in the tube is necessary. If you have questions, please call customer services 888.420-0990 or email support@familywellnessbox.com
- Remove the tube and place it in the sealable clear bag that it came in and carefully seal it.
- If parties other than the person that ordered provide a sample, please write their name and date of birth on the Vacutainer
- Your results will arrive via email from PL Consultants and will require that you create an account to obtain your confidential results. A code will be sent to you for registration and can be sent via text or email. If seeking results on a cell phone it will be easier to have the code texted to you for use in registering for your account. If results are not received within 2-3 days, please check your junk folder and call customer service.
- Place the sealed bag into the enclosed white return envelope, remove the adhesive protection strip and seal it.
- Place the return shipping label on the envelope side that was sealed positioning the top of the label over the sealed lip of the envelope.
- Deliver the envelope to the nearest UPS shipping location which can be found through this UPS location link or visit www.ups.com/dropoff.
- You may dispose of the remaining urine in the toilet and place the cup in the trash.
